Non-Invasive (Non-Surgical) Treatment of Stress Urinary Incontinence with the Laser Er:YAG SMOOTH

Every fourth woman in the world suffers from urinary incontinence of different severity (according to the World Health Organisation (WHO). About half of these women suffer from stress urinary incontinence which is involuntary leakage of urine when a person participates in some type of physical activity: such as when running, climbing stairs, lifting something, coughing, sneezing, exercising, making love, etc. This considerably limits active life of women.

In Lithuania there is about a million of working-age women (according to Lithuanian Department of Statistics). About 265 thousand of women cannot hold urine and as a result of this problem about 8 thousand of them cannot work. This is half of the population of Vilnius or the whole population of Kaunas. Often women do not even tell their gynaecologist that they suffer from this problem. Most often women think that there exists no effective treatment method, they are ashamed and think that these are changes which appear when they get older.

For women this becomes not only a medical problem but also a social problem. Even though this is not a condition on which depends the life of a woman, it makes serious harm to the quality of her life. Women refuse their favourite sports and active leisure time; this problem regulates their schedule, they feel insecure and uncomfortable, the quality of their intimate life suffers as well.

Stress urinary incontinence develops when the supportive function of pelvic floor muscles and fascia get weaker (because of age, after giving birth, after gaining weight, because of congenital peculiarities of the connective tissue, because of heavy work, during menopause, in case of chronic constipation, chronic respiratory diseases, trauma of the minor pelvis, etc.) and the position of the urinary bladder and internal female genitals changes. This causes the change of pressure in the urethra and bladder resulting in increased pressure in the abdominal cavity (e.g. when coughing) and pressure in the urethra becomes smaller than in the bladder and urine leaks involuntarily.

Problems related to urinary incontinence:

  • Involuntarily leakage of urine when coughing, sneezing, running, dancing, exercising, lifting something, laughing, during sexual intercourse
  • Need for daily pads
  • Fear of surgical operation, uncertainty
  • Discomfort, lack of confidence, social isolation, “a sign of old age”, may cause depression
  • skin diseases due to skin irritation with urine

What is the principle of the treatment of stress urinary incontinence with the laser ER: YAG (SMOOTH)?

The laser technology IntimaLase IncontiLase TM and TM (SMOOTH) in SUGIHARA Clinics is the only technology in the world which does not damage the vaginal mucosa and since the treatment is being done in the depth of 10-11 mm other internal organs of a woman (cervix, uterus (myomas), ovary, etc.) are not affected. The treatment is based on photothermic effect and is performed in SMOOTH mode without cutting, sewing, etc.

This treatment is for stress urinary incontinence of mild and moderate severity. It brings positive effect in case of mixed urinary incontinence.

Thanks to photothermic effect of the laser ER: YAG 2940 nm (SMOOTH):

  • the collagen of the front walls of the vagina, the walls of urinary bladder, vestibular part of the urethra and endopelvic fascia contract (the position of the urinary bladder and urethra changes which results in complicating involuntary leakage of urine)
  • the synthesis of new collagen is stimulated (activation lasts up to 6 months; therefore, the therapeutic effect is achieved within 6 months)
  • cardiovascular neogenesis is being stimulated, i.e. production of new blood vessels (tissue nutrition is improving)
  • All these effects create better support for the urinary bladder and other pelvic organs; therefore, symptoms of stress urinary incontinence disappear or significantly decrease.

Advantages of the laser treatment of urinary incontinence:

  • Non-invasive (incision is not needed)
  • Not painful, anaesthesia (pain relief) is not needed
  • Special preparation is not needed
  • Secure (there is no area of tissue damage)
  • One treatment lasts for 15-30 min.
  • Most often two treatments is needed with one-month gab between them but it might be that up to four treatments are needed (depends on severity of the pathology)
  • After the treatment it is allowed to immediately return to the daily work
  • Improvement will be noticed immediately after the treatment and it will strengthen within the coming six months (high satisfaction rate)

What are the results of the non-invasive laser treatment of urinary?

  • 94% of the women mentions significant weakening of symptoms of urinary incontinence
  • For 70% of the patients symptoms of urinary incontinence disappears completely (they remain completely “dry” for 120 days after the treatment)
  • It brings positive effect in case of mixed urinary incontinence
  • No side effects noticed
